By signing this appointment and consent form, I hereby acknowledge, consent, and agree to all of the following:

  • There is a small risk of burn from the welding spark.
  • All jewelry is capable of causing accidental injury.
  • If under the age of 16, you are accompanied by your guardian.
  • If you are a parent or guardian, you understand that permanent jewelry can be a choking hazard. You are responsible for supervising and monitoring your child's permanent jewelry.
  • The welding spark can be harmful to your eyes. Safety eyewear is provided.
  • This jewelry can be cut off in case of an emergency.
  • All sales are final. If you decide you don't like bonded jewelry, a clasp can be added for an additional charge.
  • If your jewelry breaks, it can be re-bonded free of charge as long as you keep your chain.
  • Permanent jewelry is not indestructible, it still requires cleaning and care.
  • Moss & Metal and So Pretty Beauty Studio are not held liable in cases of emergency.
